- Your complete medical history and medication list: Many general health conditions are associated with visual symptoms and/or health problems. This important information will allow us to care for you as a "whole person" rather than just a pair of eyes. Please include both prescription and non-prescription medications.
- Your ID and insurance cards: Even for "routine" visits, if a medical eye condition is discovered during your examination, we can submit a claim to your health insurance for the medical portion of your examination. Also, if you have an emergency eye evaluation in the future, we will have all of the required elements in your file beforehand.
- Eyeglasses: When you bring all of your current eye wear, we are able to compare your new prescription to what you have been using and allow us to see how your eyes are changing with time. We can also evaluate the condition and fit of your current eye wear.
- Contact lenses: It is very helpful if you bring along cartons or lens packets of your current contact lens prescription. It is also helpful to wear your current lenses in to your examination.
- Children: Please bring along any referral form if a problem was found at a school screening or at the pediatrician. This will help us rule out or confirm the suspected problem more efficiently, and also allow us to communicate these findings to the referring provider. A parent or guardian must accompany any minor to their examination.
